I have updated the intake forms and require all patients to fill them out online before every scheduled appointment to ensure accurate updated COVID-19 screening information from each patient.
As I rely solely on your honesty and cooperation to be able to continue my practice during this time, I am very grateful for your compliance.
What to expect prior to treatment:
• Mandatory Covid-19 intake to be read and answered before coming into the clinic. (these will be available through my online booking site- www.winonaolsonrmt.janeapp.com and you should be prompted to fill them out at the time of booking) This includes a quick COVID-19 screening questionnaire which will be emailed to you 24 hours before every appointment.
• PLEASE KEEP IN MIND ANY FORMS NOT READ, SIGNED AND SENT BACK PRIOR TO TREATMENT, MEANS YOU MAY NOT BE ABLE TO RECEIVE TREATMENT AS IT IS MY DUTY TO OBTAIN IMPORTANT HEALTH HISTORY AND COVID-19 SCREENING INFORMATION.
Patient screening: • If no form has been filled out online 12 hours prior to appointment, please expect a phone call where your patient screening can take place. Will go over Honest patient self assessment, new procedures and any questions on the phone • Bring mask or one will be available here for purchase - $2 • Rescheduling appointment, please give 24 hours notice, as appointments are limited and I need ample time to fill your spot HOWEVER, if you are sick or present with any symptoms related to COVID-19 or any other cold or flu-like symptoms within this 24 hour period, you will NOT be charged a late cancellation fee.
